India runs one of the largest living-donor transplant programmes in the world. For international patients the process is tightly regulated: transplants are permitted from a near relative or a documented altruistic donor, with mandatory authorisation-committee approval and embassy attestation of the relationship.

Indian neurosurgical centres operate with intraoperative neuromonitoring, neuronavigation, awake craniotomy protocols and dedicated neuro-ICUs. Complex cases that would face long waits elsewhere are typically scheduled within two weeks of arrival.
