What the procedure involves
Shoulder replacement resurfaces the ball-and-socket joint and is used for advanced osteoarthritis, avascular necrosis or severe fracture. A conventional total shoulder replacement is chosen when the rotator cuff is intact; a reverse shoulder replacement reverses the ball-and-socket geometry and is used when the rotator cuff is torn or deficient, which is common in older patients with long-standing cuff disease.
The choice between conventional and reverse implants is confirmed on MRI or ultrasound assessment of the rotator cuff, not decided in advance. Reverse implants generally cost more because of the implant design and instrumentation, and this is stated clearly before surgery is booked.
